2. Submit COMPLETED Claim Form along with the following documents:

Original Hardware Receipt/Proof of Purchase
Original Claim Form
Police Report
Photos of Damaged Item
Photos of the Affected Scene
For Service Contract Claim/1 To 1 Replacement Plan
Mechanical or Electrical Breakdown
For All Risk Protection Claim
Lightning
Flood
Burglary
Armed Robbery
Accidental Damage
For Theft Protection Claim

3. Claim request for defective unit within 1Care Warranty period will be free of charge.
